1. Immunization Form (Required)
Every Union Graduate College student who takes 2 or more classes per term or 6 classes per year (full-time status) must provide their immunization records on the required form. This is a federal regulation but can cause some confusion for part-time graduate study students who take one course per term and then decide to take two courses in a particular term.
The Immunization Form needs to be completed in order to take two courses (which is considered a full-time load) in a term. Therefore we suggest that part-timers complete the Immunization Form early in their program. This way the form is complete if and when they decide to take two courses in a particular term.

We strongly encourage part-time students who plan to take more than one graduate course per term at any point during the MBA program, to submit an Immunization Form.
2. Insurance Waiver Form (Required)
Only full-time Union Graduate College students are eligible for insurance and are automatically enrolled and charged for health insurance by the College. If you are already enrolled in a health insurance program, you must complete the enclosed waiver form in order to opt out of Union Graduate College’s coverage.

3. Permission to Release Educational Information to Union College (Required)
This form is generally completed at or before you register for your first graduate course(s). The purpose of this form is to allow Union Graduate College to release your ID to Union College in order to secure a student ID card and on-campus parking pass.
4. Application for Computer Account (Required)
This form is completed when you register for your first graduate course(s). Once you have submitted this form and it is processed by ITS, Union Graduate College Registrar’s office will call you with your password. You will have the option of changing the password when you have access to the network.
